If I happen to make a mistake, the revision will be taken care of for free. Otherwise, my rate always includes 2 rounds of pickups. Anything beyond that will need to be discussed to determine your budget for ongoing revisions.
I am not able to offer in perpetuity commercial rights but I am happy to agree to a buyout for a specific period of time and then you are welcome to use the project organically beyond that for as long as you'd like.
